Appointments and Cancellations
I take our relationship very seriously. When an appointment is scheduled, that is my commitment to be fully present with you as we engage in our work together. As I set aside time for you via an appointment, I require 24 hour notice should you need to cancel. You will be charged for the session for cancellations that do not occur with 24-hour notice. Please ensure that you call the office as soon as you are aware of the need for cancellation. I will make every attempt to work with you. Should I not be able to take your call, please leave a message.
Destin Services are "self-pay" and fees are due at the time of service. Insurance is not handled through my office. I will, however, provide you with a detailed receipt with the necessary diagnostic codes that you can submit to your insurance company for reimbursement. However, know that your insurance is a contract between you, your employer (where appropriate) and your insurance company. As such you are responsible to know your plan and the services for which you qualify. Contact your insurance company for more information.
*For your convenience, all major credit cards, cash, checks and debit cards are accepted.
Any oustanding balances that are beyond 60 days may be subject to legal remedy in order to satisfy the account. Please make certain to stay in contact with me in order to ensure that payment arrangements have been made. If such action becomes necessary, all legal costs will be included in the claim.
A returned check will result in a $25 fee along with a cash payment for the session. More than two returned checks will result in sessions being credit card or cash only.
